There are several ways to obtain a WCA Home application form:

1. Send your request via mail (WCA Home, 134 Temple Street, Fredonia, New York, 14063), and we will mail an application form to you.

2. Phone (716-672-7961) or Fax (716-672-3496) your request. We can send an application to you via mail or Fax.

3. Download the application form from this page. This option requires that your computer have the free Adobe Acrobat Reader software. (Most computers have this software installed when they are purchased. If yours does not, click here to go to the Adobe site where you may obtain your free copy.)

Once you have downloaded the application, you may view it on your computer and print a copy on your printer.
